About this Form

You must complete a certificate for each recipient you paid interest to with RWT deducted during the year.

When to use this form

An IR15 or an alternative form approved by us must be completed after 31 March each year.

What you will need

• recipient’s IRD number and details
• total interest liable for resident withholding tax
• resident withholding tax deducted
• account number
• payer’s details and IRD number
