This form records an employee’s KiwiSaver membership and chosen employee contribution rate. It is kept by the employer, not sent to Inland Revenue.
How to use it
Complete and sign the form, give it to your employer and keep a copy. The employer should update payroll deductions and retain the form with employment records.
