This form is limited to the first two months after Inland Revenue receives the first contribution. After that, the KiwiSaver scheme provider handles withdrawal applications.

How to use it

Complete the personal and application details, explain the hardship or illness, attach the required evidence and submit within the two-month window. If the window has passed, contact the scheme provider.
