About this form

IR120 helps IRD confirm a child’s recognised care arrangements when the people involved disagree or IRD needs more information.

What to provide

• Details of where the child stays overnight and other ongoing care arrangements
• Complete the questionnaire and attached care calendar
• Include supporting evidence if the care arrangement is disputed
• Tell IRD about care changes within 28 days where possible
